It is a pre-formed space maintainer used in pediatric dentistry to prevent pre-mature loss of space and prevent first permanent molars from moving mesially with the premature loss of second primary molars. The example shown is a crown with a distal extension segment soldered to the crown.

Indications:

• Premature loss or extraction of the second primary molar prior to the eruption of the first permanent molar.
• Advanced root resorption and periapical bone destruction of the second primary molar prior to the eruption of the first permanent molar.
• A primary second molar with advanced caries that is not restorable
• Ectopic eruption of the permanent first molar
• Ankylosis of the primary second molar.
